This build is designed to kill the zerg as fast as possible using 2 immortals and Zealot charge. It is not possible to transition from this because you are removing probes from gas and putting them into minerals to maximize zealot production. You can trick the Zerg by putting a pylon on your third (he would probably have vision there) so he does not expect the attack so early.
It is important to micro the prism and save immortals as much as possible without forgetting to warp in zealots every time possible.
Warp in Zealots half way Zerg's base, warp again at natural. This should hit the Zerg's natural with 12-14 zealots 2 immortals and charge at 5:16-5:20 minute mark, you should be able to do a lot of damage and just kill the zerg right there.
Don't forget to deny vision with a salker, if the number of gates is scouted before you go out of your base he is going to be able to prepare a good defence.
